Results of a randomized trial from Spain comparing the Mediterranean diet with either extra-virgin olive oil or mixed nuts to a low-fat diet on breast cancer found a positive effect on the primary prevention of breast cancer with the Mediterranean diet with extra-virgin olive oil.

After a 4.8 year follow-up it was found that one woman per 1000 in the Mediterranean diet group with extra-virgin olive oil had breast cancer as compared to 1.8 in the Mediterranean diet group with mixed nuts as compared to 2.9 in the low fat diet control group. Olive oil is a clear winner.
