New adult patient intake (1-1/2 hour) that includes comprehensive evaluation, physical exam, and treatment protocols - $325.00

Extended new adult patient intake (2 hours) - $400.00

New pediatric patient intake (usually 1 hour) that includes comprehensive evaluation, physical exam, and treatment protocols - $225.00

Extended new pediatric patient intake (2 hours) - $400.00
Subsequent and follow-up visits are 1/2 hour - $100.00; 45 minutes - $150.00; full hour - $200
Prescribed medications, labs and any additional treatments are separate fees.

Phone consultations
For simple questions that require 5 minute discussions there are no charges. For more complex decision making calls, the hourly rate applies.

Regarding Payment
Payment for physician or other healthcare practitioner services, dispensary medicines,
diagnostic tests, and all other items is required at the time of services rendered unless
other arrangements have been made with the business manager. If pressing financial hardship should interfere with your ability to pay for services, please discuss this with our business manager before your initial visit. Every effort will be made to arrange a flexible payment schedule to meet your needs. Payment can be made by cash, check or credit card (American Express and debit cards are not accepted).

Regarding Insurance
Naturopathic medical care is not currently covered by HMO’s or Medicare in New York State. We shall continue to keep abreast of any changes that may increase access to care for patients with insurance and we will keep patients posted of any change. We provide a superbill receipt or medical receipt coded with the services rendered so that you can submit to your HMO or insurance company for direct reimbursement.

Patients seen in the NY office can submit for insurance reimbursement for nutritional counseling and massage services.

Cancellation Policy
Cancellations for all appointments must be made within 48 hours of the scheduled appointment. Messages can be left at (845) 358-8385. If the appointment is not cancelled within 48 hours then you will be charged the full fee.
