Testing and treating osteoporosis. The current "gold standard" test for diagnosing osteoporosis in patients without an osteoporosis fracture is the dual energy x-ray absorptiometry (DXA). Risk factors for osteoporosis fractures may include but are not limited too long-term use of certain medications, especially glucocorticoids, anticoagulants, anticonvulsants, aromatase inhibitors, cancer chemotherapeutic drugs, and gonadotropin-releasing hormone agonists. Although these fractures can be present in any bone they occur most often in the hip, spine, and wrist.

The American College of Physicians recommends that clinicians offer pharmacologic treatment with alendronate, risedronate, zoledronic acid, or denosumab to reduce the risk for hip and vertebral fractures in women who have known osteoporosis. The also urge against using menopause estrogen therapy or menopausal estrogen plus progestogen therapy or or raloxifene for the treatment of osteoporosis in women. According to evidence cited in guidelines from the ACP published in Annals of Internal Medicine, “zoledronic acid is associated with hypocalcemia” and influenza-like symptoms, and “bisphosphonates are associated with mild upper gastrointestinal symptoms” but “[e]vidence is insufficient to associate bisphosphonates with increased cancer risk, because studies report conflicting results.”
