In a study of participants who tested positive for anti-citrullinated protein antibody (ACPA) and no signs of arthritis, 80% had tenosynovitis.
The study is suggesting that in people who have already developed autoimmunity specific to RA, but have not progressed to arthritis they have mild inflammatory lesions already present.
The idea is that patients who test positive for ACP may initiate treatments before arthritis starts.
The participants in the study underwent a gadolinium-enhanced MRI and the images were assessed for erosions, osteitis and synovitis. They were also assessed for tenosynovitis.
“Tenosynovitis was the most frequent pathology in ACPA-positive individuals, "exceeding synovitis and osteitis and afflicting the majority of subjects."
Tenosynovitis is associated with the later development of RA. None of the healthy controls in the study displayed any signs of having tenosynovitis.
The findings in this study are able to identify a precursor to developing autoimmune but, not clinical features of RA.
The presence of tenosynovitis was also associated with higher instances of arthralgia and morning stiffness. This reveals a pathological change in the pre-phases of RA.
This can lead to the most earlier prevention and treatment of RA.
